Heritage Health and Nebraska Total Care 

It’s time to take control of your health – and this is the place to start. Your new health insurance plan offers comprehensive medical care and benefits. It also includes valuable programs, educational tools and support.

You will receive a New Member Packet in the mail. It is a large green envelope and has important information about your plan. Please watch for this packet.

This page will help you understand your health plan. It has all the details you need to help manage your health. You can also find tools to take action.

Your next steps with your new plan:

Create an account on our secure online Member Portal. You can manage your health benefits, change your primary care provider, contact a plan representative, see authorization approvals, view claims, see your healthy rewards program balance, and more.

Download a digital ID card. This is available in your Member Portal and the MyNTC mobile app.

Complete a Health Risk Screening. This is available on your Member Portal. You can also complete it by calling 1-844-385-2192 (TTY 711). It is best to complete it within 30 days. 

Read your Member HandbookYour handbook tells you about your benefits and how to use the services and programs available.

If pregnant, complete a Notification of Pregnancy form. This is available in your Member Portal.

Choose your doctor. Sign in to your Member Portal account to choose your doctor, or call us at 1-844-385-2192 (TTY 711). You can also use our Find a Provider tool to search for a doctor who is located in your area.

Visit your doctor

Visit your doctor. After you choose your doctor, set up an appointment for a check-up right away. This is your new medical home.

Take the Social Needs Self-Assessment. Identify your needs so that we can connect you with community and social service programs.

MAKING APPOINTMENTS 

You should be able to schedule an appointment with your PCP and get medical care when you need it. You may have to wait a little longer to get in to see certain other types of providers, like specialists. Call your health plan if you can’t get an appointment in a timely manner.
