Heritage Health (Medicaid):

The Drug Rebate Program covers prescribed drugs only if the labeler has signed a Rebate Participation Agreement with the Secretary of Health and Human Services, Centers for Medicare and Medicaid Services (CMS).

Those drugs manufactured by companies (labelers) participating in the federal Medicaid rebate program are reimbursable under Medicaid.

Only one NDC with a single CPT or HCPCS code should be added on each line of a claims submission.
