Tips for Submitting Claim Reconsiderations and Appeals

Heritage Health (Medicaid):

When submitting a claim reconsideration to Nebraska Total Care, please use the identified reconsideration form (PDF) as the face sheet to the reconsideration packet.  It is important that this form be the top sheet submitted with a reconsideration.  Additionally, when submitting a claim appeal to Nebraska Total Care please use the identified appeal form (PDF) as the face sheet to the appeal packet.  It is important for claim reconsideration and claim appeal processing that these are the lead documents submitted.  Additionally, when reconsidering or appealing a claim, a copy of the original claim is not required to be in the packet.
